Facility Request Form

In order to reserve a room at the church or to have your offsite, church-sponsored event put on the primary church calendar, please complete the following form.  You will be emailed a copy of your request upon its completion.  You may request specific rooms, but, because of space considerations, Christ Church may not be able to provide those specific rooms. If you would prefer not to use this online form, you may complete this paper form and turn it into the church office.
